HEALTH INFORMATION LETTER 2023-2024
Health forms are available through and should be returned via Magnus, which is accessed through LREI Connect. New families will be given access to LREI Connect (and Magnus) in July. Please read the above letter for detailed information on the forms and upload process.
Medications:
We will have a nurse on this trip to attend to student medical needs and administer medications.
- Students are not permitted to carry or administer any medications with the exception of Epi Pens and Inhalers for emergency use with written permission from their medical provider.
- High school students with Epi Pens and Inhalers must be designated as “independent” for these medications on their forms due to the level of independence they will have throughout the school year.
- Our nurse will have access to standard OTC medications, and can administer them with parental consent in Magnus. There is no need to pack the following medications: Tylenol, ibuprofen, benadryl, bacitracin, hydrocortisone, tums.
- If your child will need any other medications while on the trip, please have their medical provider complete the attached form. This form must be co-signed by a parent. Please upload this form to your child’s Magnus account.
- Homeopathic remedies and supplements will not be administered.
- Medications brought from home must be delivered to school in advance. Please send only the amount of medication/number of doses needed for the trip. All medication must be in the prescription bottle/original packaging. Ava will be at LREI to collect medications on Tuesday 9/5 from 9am – 1pm. Please email Ava (adawson@lrei.org) to coordinate if a different date/time is needed.
