Fifth and Sixth Grade Trip . . .
- In October, fifth and sixth graders will be heading up to the Ashokan Center for our annual outdoor education experience. The cost of the trip is $350. It is our expectation that all students will participate in this trip. If you have specific questions about payment, please do not hesitate to contact Ana Fox Chaney. Because the trip happens early in the year (10/19-10/21), we ask that you complete the attached Trip Permission and Ashokan Health forms and return them with your LREI Health forms. Ashokan requires us to submit these forms in and they will not accept the LREI Health form as a substitute. In order to dispense any OTC or prescription medications a doctor’s signature is required on the Ashokan Health form. The folks at Ashokan are very particular about this documentation. Please note the following:
- For those of you who have your annual doctor’s visit in August or after the start of school, we ask that you still have your doctor’s office complete the form if you want/need your child to receive OTC or any required prescription medications as the form does not require an actual check-up.
- If your child will not need any OTC or prescription medications, you can indicate “No” on the form and sign the bottom of the Health Record form. In this case, a physician’s signature is not required.
- We also ask that you make sure that the LREI Health form is completed and returned to us before the start of the school year. Making sure that you have submitted these forms by the requested dates helps us to ensure that your child has the best possible experience. Please do not hesitate to contact Ana Fox Chaney if you have any questions. The August mailing will include additional information about the trip.
